ALF Psychosocial Rehabilitation Service

Assisted Living Facilities (ALFs) are key to TMCSCC  care coordination and care transition efforts. To enhance the ALF experience, TMCSCC introduced our Assisted Living Facility Behavioral Health Program (ALF BH), which provides Psychosocial Rehabilitation (PSR) services, for residents at ALFs.  

PSR combines independent living and social skills training, pre-vocational and transitional training, social support, and network enhancement to recipients and their families. TMCSCC recognizes that residents may experience feelings of loneliness, depression, anxiety, or stress and by prioritizing mental health support through PSR our ALF BH program creates a nurturing environment that fosters emotional resilience, social connection, and a sense of belonging.
